If you wish to make an application for a school place for your child please download, read and complete the following documents.

Your application form will need to be supported with the documents below.

· Psychological Assessment Report and clearly stating a diagnosis of a physical disability and also recommending a special school placement. Parents are advised to refer to the Admissions Policy for further details regarding this criteria.

· Full Birth Certificate

· Letter of eligibility for special school from NCSE

· Document for Proof of Address (eg utility bill or letter from government department)

All other available professional reports must also be submitted such as:

· Speech and Language Assessment Report (including FEDS report)

· Occupational Assessment Report

· Medical Report

· Current School Report and IEP

Parent(s)/Guardian(s) are advised that these documents must be supplied before your child can be considered for admission to Enable Ireland Sandymount School.
